What to Expect During Your Stay at Crescent Care of Elgin

Every stay begins with Comfort + Care. Whether you are recovering after a hospital stay, rebuilding strength through rehabilitation, or transitioning to ongoing care, our team will work to make your experience clear, supportive, and comfortable.

We understand that moving from the hospital to a skilled nursing center can bring questions for guests and families. From arrival through discharge planning, our team coordinates nursing, rehabilitation, dining, activities, and social services around your individual needs and goals.

Arrival and Getting Settled

When you arrive at Crescent Care of Elgin, our nursing team will welcome you, help you settle into your room, complete an initial assessment, review your medications and physician orders, and address your immediate care needs.

We will also review your hospital records, medical history, and any special care needs. This information helps the team begin developing an individualized care plan centered on your safety, comfort, recovery goals, and daily routines.

You and your family are encouraged to ask questions and share the preferences that will help you feel more comfortable during your stay.

Within the First 24 Hours: Evaluations, Introductions, and Care Planning

During your first day, you may begin meeting additional members of your care team. Depending on your needs, this may include physicians or advanced practice providers, physical, occupational, or speech therapists, social services, dietary team members, and other support staff.

Therapy evaluations may also begin based on your condition and physician orders. These evaluations help the team understand your strength, mobility, daily living abilities, communication, cognition, swallowing needs, and other areas that may benefit from rehabilitation.

Together, we will begin identifying meaningful goals—whether that means returning home safely, improving mobility, rebuilding independence, or managing an ongoing health need.

The First Few Days: Establishing Your Routine

As your care plan takes shape, you will begin settling into a daily routine. Your schedule may include nursing care, medication management, therapy sessions, meals, rest, visits with family and friends, and opportunities for social or recreational activities.

Depending on your needs, care may also include wound care, IV therapy, or other skilled services identified in your care plan. Your team will monitor your progress, communicate changes, and adjust the plan as appropriate throughout your stay.

Daily Care and Support

Each day is designed to support progress toward your individual goals. Nursing support is available around the clock to administer medications, monitor changes in condition, address medical needs, and coordinate care with the broader interdisciplinary team.

Rehabilitation may also be an important part of your stay. Physical therapy may support strength, balance, walking, and mobility. Occupational therapy may focus on daily activities such as dressing, bathing, grooming, and preparing for routines at home. Speech therapy may address communication, cognition, or swallowing needs.

Your therapy schedule and treatment plan will be based on your individual needs, progress, and physician orders.

Dining, Activities, and Everyday Comfort

Meals are planned with attention to dietary needs, physician orders, and personal preferences whenever possible. Dietary team members may work with you and the care team to support nutrition and help make mealtimes comfortable.

Guests may also enjoy recreational and social programming, lounges and activity areas, gardens or open-air courtyards, and spaces for visits with family and friends. These everyday experiences can help create connection, familiarity, and balance throughout recovery.

Communication and Care Updates

Open communication is an important part of the care experience. We welcome questions from guests and, with the guest’s permission and as appropriate, the family members or others involved in their care.

Throughout your stay, the team may provide progress updates, hold care plan meetings, and begin discharge planning discussions. These conversations are intended to help you understand your care, participate in decisions, and feel informed about what comes next.

Planning for What Comes Next

Discharge planning begins early in your stay. While you focus on day-to-day recovery, your care team will also help prepare for a safe and appropriate next step.

For some guests, the goal is returning home with support from family, caregivers, or home health services. For others, the next step may involve assisted living, independent living, or long-term care. The team will help evaluate your progress, discuss available options, and coordinate transition needs.

Our goal is to help you leave Crescent Care feeling informed, prepared, and confident about the plan ahead.

Helpful Tips for Your Stay

Consider bringing comfortable clothing, supportive shoes, and a few personal items that help your room feel familiar. Please label clothing and personal belongings whenever possible, and keep frequently used items within easy reach.

Most importantly, ask questions whenever something is unclear. Our team is here to help you understand your care and feel supported throughout your stay.
